Achieving the objectives

It is important that the Supporting Families Team is able to track the progress of the overall cohort to ensure we are able to make timely claims for payments as families achieve their outcomes.

 

Submitting a Payment By Results (PbR) claim

In order to submit a claim, the family must show ‘sustained and significant progress’ in accordance with the guidance provided in the Supporting Families Guidance.

Supporting Families ‘key workers’ will be asked for regular updates on progress and should notify the Supporting Families team. The team will also monitor its data sets to check progress.

It is the responsibility of the Supporting Families Team to assess outcomes and submit claims to the Council’s internal audit team for validation.

 

Case closure

Once a successful claim has been made the family will cease to be part of the Supporting Families programme although they may continue to work with local agencies and organisations.
