The service is provided to anyone registered with a Milton Keynes General Practitioner (GP) and/or who lives within the boundaries of Milton Keynes Council and who has been assessed as meeting the need for equipment.

Individuals or their families/carers can find out what equipment may help by

• using the Equipment Self Assessment tool to look at possible solutions
• requesting an assessment of need (for which equipment may be a solution) from Adult Social Care
