Examples of the evidence needed to support a claim for benefit are shown below:
Proof of Identity and Proof of National Insurance Number
We require a minimum of two pieces of evidence from you (and your partner if you have one), to prove your identity and National Insurance Number. Examples include:
- original birth certificate
- driving licence
- wage slips from current employer
- passport
- credit card
- divorce or annulment papers
- benefit entitlement letter
- gas, electric or water rates bill
- life assurance policy
- letter from a solicitor, doctor, social worker, probation officer or the Inland Revenue
- NHS medical card
- marriage certificate
- bank statement (should be dated within last 4 weeks)
- HM forces certificate of employment
- EC or EBA national identity card
- Home Office acknowledgement letter
