Achieving the objectives, submitting a payment by results claim, and case closure
It is important that the Strengthening Families Team is able to track the progress of the overall cohort to ensure we are able to make timely claims for payments as families achieve their outcomes.
In order to submit a claim, the family must show ‘sustained and significant progress’ in accordance with the guidance provided in the Financial Framework (March 2015).
Strengthening Families ‘key workers’ will be asked for regular updates on progress and should notify the Strengthening Families team. The team will also monitor its data sets to check progress.
It is the responsibility of the Strengthening Families Team to assess outcomes and submit claims to the Council’s internal audit team for validation.
Once a successful claim has been made the family will cease to be part of the Strengthening Families programme although they may continue to work with local agencies and organisations.
