We aim to complete timely assessments to ascertain whether adults have eligible social care needs. The team work specifically with Older People and with adults who have Physical Disabilities. We are looking for experienced and committed professionals to join our team. We are a large and fast paced team that offers excellent management support and a wealth of knowledge and experience within the team. You will be completing initial assessments, re-assessments, safeguarding investigations (with the relevant training and support). As a worker within the team you will be working on rotational duty doing initial assessments and crisis intervention work. When not on duty you will be working on long term complex cases that you have picked up on duty providing greater continuity of service to our service users and, we believe, a more rewarding experience for you as the social worker.
We aim to undertake thorough, meaningful and timely reviews of adults with eligible social care needs who are supported to live in the community and those who are placed in residential and nursing care homes. We are a large and fast paced team that offers excellent management support and a wealth of knowledge and experience within the team. You will be completing reviews, addressing depleted capital issues, court of protection work and safeguarding investigations (with the relevant training and support). We are looking for social work assistants to join our team and help to ensure that those people supported by Milton Keynes Council are receiving appropriate and good quality services.
The DoLS Team is based at the newly refurbished Civic Offices in Milton Keynes. It is made up of a variety of experienced Best Interest Assessors and Community Liaison Officers. The team’s main function is to act as the Supervisory Body for Milton Keynes; co-ordinating, undertaking and administering DoLS assessments so that the statutory duties are maintained and appropriately managed.
Reablement and Hospital Discharge Team
We are a fast paced team where no day is the same. We have two functions in our team which includes supporting safe and timely discharges from hospital and supporting people to be discharged from our reablement and recuperation services in the community. We are a team that embraces challenge and our key ethos is supporting people to maximise their independence in their own homes. We are very fortunate to work as part of a joint health and social care team and therefore are able to collaboratively work together to meet the needs of the people we work with.
Community Learning Disability Team
We are a fully integrated multi-disciplinary Community Learning Disability Team. We support people with a learning disability to live their lives as independently as possible and support them to reach their full potential. We are looking for people with experience in learning disability and mental health but would like to hear from anyone with experience in either client group. We support a variety of people with a diverse range of outcomes including those with complex needs meaning that our work is interesting, challenging and variable on a day to day basis.
Mental Health and Autism Team
We are a growing and developing team, and our work is challenging but rewarding. As a social worker in the team you will be responsible for completing Care Act Assessments and reviews. We provide social supervision and produce social circumstance reports. You will be part of a duty rota, and will carry a case load of often complex cases. Our emphasis is on enhancing the recovery and wellbeing of those that access our service.
