Referrals to the Palliative Care Team can be made either by telephone, letter or fax during working hours. The palliative care team responds to enquiries and requests for help from many sources, including directly from patients and/or their families.
The aim of palliative care is to improve the quality of life of patients and their families, affected by incurable illness and address physical, psychological, social and/or spiritual concerns. The team provides specialist palliative care support and aims to provide: pain and symptoms control, emotional and psychological support, spiritual care, carer support, help with planning future palliative care, support with end of life care, advice and signposting, training and education of staff in palliative and end of life care.
