Milton Keynes Council Environmental Health team is using a Remote Assessment Form (PDF, 153KB) to assess and prioritise food businesses for an onsite visit. You will have received an email or letter to advise you that you are due an intervention and requesting that you visit this page to complete the form.

The answers provided in the Remote Assessment From will also be used to facilitate the targeting of what to focus our attention on at the subsequent onsite visit. If you are handling and/or preparing food from home, then please complete the Home Caterer Remote Assessment Form (PDF, 158KB).
