You must submit the following documents with your housing online application.  The documents can be uploaded to your housing online customer account under ‘my saved and submitted applications’ 

Please ensure all documents are upload individually and named, due to the upload limit they cannot be uploaded as one large document.  We will accept photos of documents providing it is clear and all information can be easily read.    

Please ensure all the above documents are up to date and are uploaded with your application before submitting it to us. Depending on your circumstance you may be asked to provide further information to support your application as part of your assessment

Milton Keynes City Council Secure Tenant

If you hold a secure tenancy with Milton Keynes City Council, you will need to supply the following documents with your application:

• ID for household members
• Last three months of bank statements , we require all accounts held for all adults on the application to assess future affordability
• Proof of pregnancy (if applicable) 
• If you have a medical condition that is affected by your current housing, please provide supporting documentation (If adaptions are required, you will need to submit a supporting OT report) 

You will also need to complete the following documents, which you can download here and then upload to your housing online customer account under ‘my saved and submitted applications’. 

• Income and expenditure (I&E) declaration form  
• Third-Party consent form (if applicable)  
• Consent form 

All other applicants

Eligibility and qualification evidence: 

• ID for all those on the application 
• Immigration status documents / share code where applicable 
• Last three months of bank statements, we require all accounts held for all adults on the application 
• Proof of any benefit entitlement 
• Last three monthly or six weekly wage slips. If self-employed, last twelve months statement of accounts 
• Proof of your National Insurance Number 
• Proof of pregnancy (if applicable) 
• If you have a medical condition that is affected by your current housing, please provide supporting documentation (If adaptions are required, you will need to submit a supporting OT report) 
• Photograph of applicant(s) only, either passport sized photo or digital photo 

Local connection evidence - Please supply one of the following:

• Utility bill/ council tax showing your 3-year connection 
• Utility bill/ council tax showing families 5-year address connection and birth certificates evidencing the family relationship 
• Evidence of a continuing caring responsibility 
• Proof of employment such as a contact of employment, wage slips, formal offer letter 
• Discharge paperwork from Armed forces or service records
